YOUR DATA AND PRIVACY POLICY:

1. Under GDPR we are required to advise clients about how we process your data and inform that LEWis BRAND Ltd is the data controller. If you wish to contact us, please do so via orthotics@lewisbrand.co.uk

a) As part of the client record, the clinic is required to retain information for the purpose of consultation, examination, recording treatment notes. This includes basic name, birth date and address information and a medical history relevant to orthotics to provide you with healthcare services. The legal basis for us to process your personal data is the contract with you.

b)All paper files and information therein may be entered onto a digital record software (Cliniko) and remain on computer for as long as the patient remains a patient in our clinic and thereafter for a period of eight years.

c) All information recorded will be treated as confidential, and will not be given to any other person/organisation without the consent of the patient concerned.

d) Information will be held manually and electronically and files will only be accessible by staff at the clinic who are directly involved in the data entry and processing of patient records.

2) It may be required that your practitioner corresponds with your GP or another healthcare professional to act in your best interest. This is considered best practice for patient well-being. You are entitled to withhold consent if you do not want us to communicate with other professionals.

3) You are entitled to exercise your rights under GDPR with regards to your data for example to see your notes and copies of any correspondence we send.

How we use your Personal Information (Data Protection):

In so far as the Services involve us in collecting, using, holding or otherwise processing any Data obtained from the Client which is personal data (including, but not limited to, name, address and date of birth) LEWis BRAND Orthotics shall only do so with the express consent of the Client and in accordance with any lawful instructions reasonably given by the client from time to time, and the provisions of the Data Protection Act 1998, GDPR and the rights of our client under that Act and these terms and Conditions.

LEWis BRAND Orthotics may use your personal information as follows:
a)      To provide our goods and services to you,
b)      To procure a product from a third-party provider
c)       To process your payment for the goods and services,

Governing law, jurisdiction, and complaints:
1.   The Contract (including any non-contractual matters) is governed by the law of England and Wales.
2.   Disputes can be submitted to the jurisdiction of the courts of England and Wales.
